Dog License

Dog License Application

If you own a dog that is 5 months or older and reside in Sawyer County more than 30 days, your dog is required by law to have an annual Sawyer County dog license.  Licenses must be purchased no later than March 31st to avoid late fees.  Owner must provide the following:

• Current Rabies Certificate - ORIGINAL rabies certificate is required and will be returned in an owner provided self addressed stamped envelope.  All license requests not accompanied by a return envelope will be held at the town hall for pick up.  Licenses not accompanied by rabies certificate will not be process per Sawyer County Ordnance.
• Self addressed stamped envelope (if mailing)
• Separate check payable to Town of Bass Lake 

Failure to obtain a license prior to April 1st will result in late fees of $20.00-$30.00 per dog.  NO postmarks - late fee will apply.

Spayed/Neutered = $10.00 per dog
NOT Spayed/Neutered = $15.00 per dog
